Health Care for Homeless and Underhoused People
 

Sherbourne Health Care is committed to providing quality health care services to all individuals – including those who are living in precarious housing situations or who don’t have housing at all.

 

We understand that for many who are homeless or living in shelters – accessing the health care system can be a difficult and frustrating experience. That’s why we have worked to build connections to health care by offering programs and outreach services that deliver realistic health care solutions for those who lack permanent housing.

 

Our goal is to empower you by building relationships and offering you access to a range of health care services so you can improve your health and well-being.

 

Outreach Services

Much of our work involves outreach. Through innovative partnerships with local shelters, we provide on site primary health care and advocacy services to shelter residents.

 

In addition, our Rotary of Toronto Club Health Bus visits 9 to 12 shelter stops weekly. Program staff and over 50 dedicated nurse volunteers, work together to provide on-the–spot nursing care to local shelter residents and homeless people.

 

Our Community Health Worker works off-site in partnership with Women’s Residence, a City of Toronto women’s shelter, providing health services support to homeless women who face multiple barriers to accessing health services.

 

Unscheduled Clinics

To expand the circle of care for homeless people, we also offer unscheduled care clinics twice weekly at our site. Client Resource Workers, Nurses, and Physicians work on the frontline of primary health care - facilitating access to Sherbourne programs and services and local community resources.

 

Infirmary Services

The infirmary is a 20 bed program staffed by a nurse practitioner, consulting physicians, nurses, community health workers, and a case manager to provide medical care for homeless and underhoused persons. It operates 7 days/week, 24 hours day providing recuperative and holistic health care to persons who are recovering from a medical condition in which they are expected to recover in a short period of time and do not require hospital care.
