Who are the health care providers at Sherbourne Health Centre?

Our team of family healthcare providers includes nurses, health promoters, mental health counsellors, outreach workers, dieticians, client-resource workers and physicians. As well, we can provide access to psychiatric services.

 

What services can you offer me?

At Sherbourne Health Centre, we offer a wide variety of programs and services to help you access community resources, care for your children and family, cope with trauma, address lesbian, gay, bisexual and trans health issues, and obtain much needed health support. Our team strives to offer programs and services in a welcoming, safe environment.

 

 

Is Sherbourne Health Centre open to everyone?

Programs and services are tailored to meet the many needs of the diverse communities of downtown southeast Toronto as well as the city at large. The Centre’s main client groups are individuals who identify as lesbian, gay, bisexual or trans (LGBT), newcomers, the homeless and underhoused individuals and local residents seeking primary health care.

How do I make an appointment?

New clients

There is a great demand for many of Sherbourne's programs and services. Some are currently unable to immediately accept new clients. By calling medical reception at (416) 324-4180, you can find out the status of the service you require.

 

Community Naturopathic Clinic for People Living with HIV/AIDS

To make an appointment, contact the naturopathic coordinator at (416) 324-4164 .

 

Community Chiropractic Clinic

To make an appointment, contact the clinic at (416) 324-4166.

 

Chiropody (foot care) Clinic
To make an appointment, call on Monday, Tuesday or Friday between 9:00 a.m. and noon, or between 1:00 p.m. and 4:00 p.m. at (416) 324-5701 x 5701.

What are your clinic hours?

Urban Family Health Team Clinic (serving homeless and underhoused from the City of Toronto and all others from the southeast Toronto catchment area—the area bounded by Yonge Street and the Don River to the east, and between Bloor Street and Lake Ontario to the south.)

What if I can’t attend my appointment?

If you cannot make your appointment, please contact medical reception at (416) 324-4180 at least 24 hours before to cancel. To cancel an appointment with the naturopathic, chiropractic or chiropody clinics, please contact the clinic directly.

  

It is important that you bring your OHIP card with you to every appointment. However, if you do not have an OHIP card, you can still receive Sherbourne Health Centre services free of charge.
