General Primary Health Care
At Sherbourne Health Centre, we are
committed to delivering primary health care services that meet your unique and
individual circumstances. You can access services for yourself, children, and
other family members.
Services Offered
Our primary health care team uses an
interdisciplinary approach to help clients face a broad range of issues from
health and wellness promotion to direct service – all in a safe and welcoming
environment. Services include:

and nursing care

Treatment
and monitoring of temporary and chronic illness

Preventative health care including general health teaching, routine
physicals, vaccinations and screening for disease

We also offer comprehensive
primary health care for the lesbian, gay, bisexual, trans
(LGBT), community as well as a broad range of
programs and services designed to meet other specific health care
needs.

Below are the boundaries for the client services offered by Sherbourne Health
Centre:
- Family Health Team (General and Newcomer
Populations): Southeast Toronto (boundaries: Yonge, Bloor, Don River, Lake
Ontario)
- Family Team (LGBT and Homeless/Underhoused
Populations): City of Toronto
- Counselling: Southeast Toronto
- Health Bus: City of Toronto
- Infirmary: City of Toronto
- SOY and Parenting Network: Primarily, City
of Toronto but no geographic restrictions
- Rainbow Health Ontario: Province of
Ontario
- Chiropractic Clinic: No geographic
restrictions
- Naturopathic Clinic: No geographic
restrictions
- Chiropody Clinic: Chiropodists will
service clients who are referred by Sherbourne Health Centre's providers
Appointments and Clinic Hours
If
you require primary health care services, please make an
appointment by contacting medical reception at
416-324-4180,
as we do not offer walk in service at this time.
Clinic hours vary according to the services required.
Interpreters and special needs
If you require interpretive services
or have special needs, please inform the medical receptionist of this when you
call to make an appointment.
Fees
If you have
an OHIP card, covered services are free. Please bring your OHIP card with you
to every appointment. If you do not have an OHIP card,
but live in the local community, you can
still receive primary health care services free of charge.
Electronic Health Record
Sherbourne
Health Centre continues to be a leader in using electronic health records (EHR)
to streamline client registration and improve your overall care. Your health
data is stored electronically on highly secured computers.

Your right to
privacy is very important to us. Your personal health information is secure from
unauthorized access, copying, or use and any issues discussed with your health
