General Primary Health Care

At Sherbourne Health Centre, we are committed to delivering primary health care services that meet unique circumstances. While we are open to everyone in our catchment area (see below), our services are aimed at three distinct groups: the homeless and underhoused, newcomers and the LGBT population.

Services Offered

• Our family health teams use an interdisciplinary approach to help clients face a broad range of issues from health and wellness promotion to direct service – all in a safe and welcoming environment. Services include: 

  • Newcomers drop-in clinic
  • Well baby and child care
  • Preventative health care including general health instruction, routine physicals, vaccinations and screening for disease
  • Chronic disease prevention and management
  • Diabetes education program
  • Hepatitis C treatment
  • Women in Need Klinic
  • LGBT health care and health promotion
     

Other programs and services

We also offer comprehensive primary health care for the lesbian, gay, bisexual and trans (LGBT) communities as well as a broad range of programs and services designed to meet other specific healthcare needs.

Program Service Area
Below are the boundaries for the client services offered by Sherbourne Health Centre:

• Newcomer Family Health Team: Southeast Toronto—the area bounded by Yonge Street and the Don River to the east, and between Bloor Street and Lake Ontario to the south
• LGBT Family Health Team: City of Toronto
• Urban (including homeless and underhoused) Family Health Team: City of Toronto
• Counselling: Southeast Toronto
• Health Bus: City of Toronto
• Infirmary: City of Toronto
• SOY and Parenting Network: Primarily City of Toronto, but no geographic restrictions
• Rainbow Health Ontario: Province of Ontario
   

If you require any of the above services, please make an appointment by contacting medical reception at (416) 324-4180, as we do not offer walk-in service. Clinic hours vary according to the services required.

As well, the Sherbourne Health Centre houses three independently operated clinics managed by their respective professional colleges. To attend any of these clinics, you need a referral from Sherbourne staff who are treating you. The three independent clinics are:
 

·         Community Chiropractic Clinic: No geographic restrictions

·         Community Naturopathic Clinic for People Living with HIV/AIDS: No geographic restrictions

Interpreters and special needs
If you require interpretation services or have special needs, please inform the medical receptionist of this when you call to make an appointment.

Fees

If you have an OHIP card, covered services are free. Please bring your OHIP card with you to every appointment.

If you do not have an OHIP card, but live in the local community, you can still receive Sherbourne Health Centre services free of charge.
