At Sherbourne Health Centre we understand that for people who do not have adequate housing or shelter, it is often difficult to access health care services. Sometimes it’s due to a lack of transportation, lost or stolen Health Cards, no health coverage, limited hours of health facilities, or the lack of health services that respond to the specific health needs of those who face barriers to sufficient health care and housing.
Walking in the door of any health or social service agency can be frightening and frustrating – especially if you are dealing with a number of difficult challenges.
That’s why we have developed an unscheduled care clinic as an outreach service
to connect people who are struggling with housing issue to a variety of
community resources that are available.
Regardless of the challenges you are dealing with, Client Resources workers will meet with you and assist you in obtaining the health care services you need.
You may be referred to health care providers at Sherbourne Health Centre, as well as other social and community services that may be of benefit to you.
Some of the services offered include:
■ General health care services
■ Health care for Hepatitis C, HIV/AIDS, Foot Wounds, Malnutrition
■ Safe Sex education and testing
■ Short term crisis counselling
■ Mental Health Counselling
■
Advocacy services to assist you in obtaining social assistance, housing etc.
Sherbourne Health Centre’s unscheduled care clinic runs twice weekly – Tuesdays and Fridays - every week. It is serviced by Client Resource Workers, Nurses, Physicians, and Medical Secretaries who work together to provide sensitive, confidential, and reliable health care services. No appointments are needed and you do not require ID or health coverage to get care.
